Fall Sports 2= 009

Winter Sports 2009-2010

Spring Sports starts on: &n= bsp; Monday, March 1^st

All student athletes must complete an eligibility process at the school athletic office EACH SEASON for EACH SPORT in which they wish to participate. <= o:p>

All forms in the SPORTS ELIGIBILITY PACKETS must be = properly filled out and filed with the athletic secretary before a student can be cleared to participate in a sport. Originals of all eligibility forms are required= . We cannot accept photocopies of eligibility forms.

A clearance card will be issued o= nly when all requirements have been met. NO ATHLETE is allowed to participate in practice until the coach has the clearance card.

3D"*" Academic eligibility

<= img width=3D9 height=3D9 src=3DPicExportError alt=3D"*"> Initial academic eligibility is based u= pon the grades and GPA from the previous semester (previous quarter for winter sports).

<= img width=3D9 height=3D9 src=3DPicExportError alt=3D"*"> The athletic secretary will check grades and GPA.

<= img width=3D9 height=3D9 src=3DPicExportError alt=3D"*"> In order to participate in co-curricular athletics students must maintain a 2.0 GPA and pass all classes. Grades are checked every 4½ weeks. Failure to meet these criteria will result in athletic probation for a period of 2-3 weeks depending upon the deficiency. Athletes will be allowed to participate in practice but will be prohibited = from participation in contests until the probation period is over and the grade = is passing.

3D"*" Sports Physicals

<= img width=3D9 height=3D9 src=3DPicExportError alt=3D"*"> Sport physicals MUST be current for the entire season.

<= img width=3D9 height=3D9 src=3DPicExportError alt=3D"*"> Students will not be cleared to turn out with a physical that expires during the regular season or playoffs. The ISD requires an annual physical. Every-other-year updates are no longer accepte= d.

<= img width=3D9 height=3D9 src=3DPicExportError alt=3D"*"> Signed originals of sports physical forms must be submitted. We cannot accept faxes or photocopies of physical forms or any other eligibility packet forms.

All student athletes are expected = to know and abide by the information contained in the Issaquah School District 2009-2010 Student Athletic Handbook

3D"*" ASB cards

<= img width=3D9 height=3D9 src=3DPicExportError alt=3D"*"> All students turning out for a sport/ac= tivity must purchase a $50.00 ASB card.

3D"*" Sports fees

<= img width=3D9 height=3D9 src=3DPicExportError alt=3D"*"> All athletes are required to pay a $85 sports fee.

<= img width=3D9 height=3D9 src=3DPicExportError alt=3D"*"> No student will pay more than $170 per school year in sports fees. Athletes participating in three sports in a sch= ool year only pay two sport fees.

<= img width=3D9 height=3D9 src=3DPicExportError alt=3D"*"> Students enrolled at PCFC MUST pay fees with the PCFC bookkeeper and turn eligibility forms in to the PCFC Athletic Office. PCFC will forward necessary infor= mation to Skyline High School .

3D"*" School fees and fines

<= img width=3D9 height=3D9 src=3DPicExportError alt=3D"*"> All school fees and fines MUST be paid prior to an athlete being cleared to play a sport.

3D"*" Sports fee refunds

<= img width=3D9 height=3D9 src=3DPicExportError alt=3D"*"> Sports fees will be fully refunded under the following conditions:

Student is cut f= rom the squad by the coach

Student quits du= e to illness or injury prior to 1st contest

Student quits du= e to family moving prior to 1st contest
