OUTSIDE CREDIT APPLICATION FORM INSTRUCTIONS

 

This form is used to gain pre-approval for the following outside credit options:

Outside Physical Education Activities

Outside Music or Art Lessons

Outside Planned Learning Experiences (Courses not available at Skyline)

 

1)      Complete the attached form.

-                     Read directions carefully and fill out all sections

-                     Direct any questions to you Counselor

-                     Make cure parent and instructors sign the form

 

2)      Return the completed form and $25 (non-refundable processing fee) to the Bookkeeping Office* (Checks made out to Skyline High School).

 

3)      Student will receive a receipt and the application will be reviewed at the next scheduled Outside Credit Committee meeting.

 

4)      Student will be notified after the Committee meeting as to the acceptance or rejection of the outside activity for credit. (Within 10 school days of the application due date.

 

5)      If approved, the student will need to submit paperwork verifying the completion of their “program”. The final evaluation and earning of credit will take place at the end of the semester during which the activity took place. 

 

 

*Important Dates for Outside Credit Approval:

Semester 1

      Pre-approval Applications due: September 17, 2007

      Completion Verification due:                 January 21, 2008

Semester 2

      Pre-approval Application due:   January 21, 2008

      Completion Verification due:                 June 2, 2008

Summer

      Pre-Approval Application due:  June 2, 2008

      Completion Verification due:     September 15, 2008

 

The following standards apply to all outside credit activities:

-         Activities must equal at least ninety fifty-minutes of time for a .5 semester credit (75 hours)

-         Evaluation of student learning should have student outcomes that demonstrate the proficiency level of skills and knowledge identified in the course objectives. The evaluation may include some combination of projects, portfolios, written tests, demonstrations, public performances, reflection papers, research papers, and/or activity logs.

 

- “Accredited correspondence programs may be taken for CREDIT (not grade) with prior counselor approval.

 

-     Varied learning activity credit courses should have equal rigor as courses in the same subject area in the high school program.
